Summit Medical Center has four separate registration areas. They are located at the main hospital, the Outpatient Imaging Center, Summit Imaging at Lebanon, and the Emergency Department.

Main Admitting

The hours of operation at Summit Medical Center’s Main Admitting are Monday through Friday, 6:00 a.m. – 6:00 p.m. All patients entering the main hospital for any inpatient stays or outpatient tests and procedures must first come through Registration. The Registration Desk is located in the Patient Lobby and may be reached at (615) 316-3570.

Outpatient Imaging Center

The hours of operation are Monday through Friday, 8:00 a.m. – 7:00 p.m. and Saturday, 8 a.m. – Noon. The Outpatient Imaging Center is located next to the hospital at 3901 Central Pike, Hermitage, TN.

Summit Imaging at Lebanon

The hours of operation are Monday - Friday, 8 a.m. – 5 p.m. and Saturday 8 a.m. – Noon. Summit Imaging at Lebanon is located in the Physicians Plaza of Lebanon at 100 Physicians Way, Suite 100, Lebanon, TN 37087.

Emergency Room

The Summit Medical Center Emergency Room hours of operation are 24 hours a day, 7 days a week. All emergency patients entering Summit Medical Center through the Emergency Room will be registered in this area. Any admissions occurring after main registration hours will also register through this area. The Emergency Room Registration may be reached at (615) 316-3602.

When arriving at Main Admitting, the Outpatient Imaging Center or Summit Imaging at Lebanon, remember to bring with you the following items:

• All active insurance cards 
• Picture ID 
• Living Will, Advance Directives, or Durable Power of Attorney

Central Scheduling

The Central Scheduling staff will work with you and your physician to arrange your appointment for diagnostic procedures at a time convenient for your schedule. Central Scheduling is open from 7:00 a.m. to 6:00 p.m. Monday through Friday. The phone number is (615) 695-7229 or (866) 695-7229.
